What is FMR?
Specific medical information that is conveniently loaded onto a keychain or necklace flashdrive.
The same information is also centrally accessible from any computer with your username and password.
Like accessing your bank account.
How can it help me?
Your medical records will be immediately accessible for any emergency situation, but also for when
you change physicians or go to a specialist. Records are contained in a portable flash drive and also online.
Why is this important?
Every patient should be responsible for their own medical records. Electronic medical records are supposed
to be in place by 2014, but when has the government ever completed something on time? Even if the
electronic medical records do come online, it is unlikely that an Emergency Room will EVER be able to access your private
doctor's office. So it's up to you to ensure that your healthcare is safe and appropriate. With your pertinent
records readily on hand, you can greatly improve appropriate care.

Will healthcare providers use FMR?
They will if they are smart! There isn't a caring, competent healthcare worker who doesn't
want the most information about your health when treating you.
Can you compensate my doctor?
Have your doctor or other healthcare provider or caregiver
enroll as an Affiliate
first with Flash Medical Records. We will then compensate them generously for enrolling you in FMR and providing us with your records.
Who needs Flash Medical Records?
Everyone! But especially persons with complicated medical histories, multiple medications,
surgeries or other procedures. Also your loved ones who may not be able to easily care for themselves,
such as Alzheimer's patients.

How do I provide my medical records?
Fill out the form with answers to as many questions as you know. This will become your Face Sheet.
All of this information is received by us electronically when you sign up. You can go back to this form and
add new information whenever necessary.
Will you include other documents?
You may also send revelant documents that we recommend be included on your flash drive:
Dictations, Lab Results, Xrays, MRIs, EKGs and other miscellaneous records.
These are the records that are pertinent to physicians and concise enough to fit
into the busy schedules of all doctors. You can
then email (recommended), fax or mail these documents to us.
How do I get enroll in Flash Medical Records and get my flash drive?
